The Lab Work We Run Before and During Peptide Medication Therapy
A prescription without lab work is a guess wearing a white coat. A woman came to see me last spring holding a printout from an online clinic. She'd been on a weekly injectable for four months, dropped twenty-two pounds, and felt a bone-deep tired she couldn't explain away with work or kids or the Texas heat. Her question was simple. Was this normal? My honest answer was that I had no way to know, because nobody had ever drawn her blood. Not before she started. Not once in four months.
That story isn't rare. It's the single most common thing I fix when patients transfer their care to Magnolia Functional Wellness here in Southlake. Peptide medications are real drugs with real physiology behind them, and the companies that developed the branded products ran enormous, expensive trials to prove it. But a prescription issued without lab work is a guess wearing a white coat. So let me walk you through what we actually order, when we order it, and what we do with the numbers when they come back.
Why We Draw Blood Before the First Injection
Baseline labs do three jobs, and only one of them is about safety.
The first job is screening. Some people shouldn't start these medications at all, and a handful of them have no symptoms that would tell you so from across an exam room. Kidney impairment, active liver disease, an unexplained elevation in pancreatic enzymes, an untreated thyroid problem. You find those on paper, not on inspection.
The second job is diagnosis. Weight gain and fatigue are the two least specific complaints in all of medicine. I've had patients arrive convinced they needed appetite suppression when what they actually had was iron deficiency, or subclinical hypothyroidism, or a hemoglobin A1c that had quietly crossed into diabetes territory. Treating the wrong problem with the right drug is still treating the wrong problem.
The third job is the one people underestimate. Baseline labs give you a comparison point. Six months from now, when your ALT comes back at 41, the only question that matters is what it was before. If it was 38, we shrug. If it was 19, we look harder. Without that first draw, every future result is uninterpretable, and you end up either chasing ghosts or missing something real.
What I tell my patients is that the panel isn't a hurdle between them and the medication. It's the thing that makes the medication worth taking, because it's how we know whether it's working on the right problem and whether your body is tolerating it.
The Baseline Panel, and What Each Test Is Actually For
Here's what we typically draw before anyone in my practice starts a peptide medication. Individual panels vary, and I'll add or subtract based on your history, your medication list, and what you're actually trying to accomplish.
- Comprehensive metabolic panel. Fasting glucose, electrolytes, kidney function (creatinine and eGFR), and liver enzymes in one draw. This is the workhorse.
- Hemoglobin A1c. Three-month average blood sugar. Tells me whether we're treating obesity, prediabetes, or diabetes, and those are three different conversations.
- Complete blood count. Catches anemia, which explains a surprising amount of fatigue that gets blamed on the medication later.
- Lipid panel. Total cholesterol, LDL, HDL, triglycerides. Cardiometabolic risk is the whole reason many of these patients are in my office.
- TSH, with free T4 when indicated. Thyroid dysfunction mimics almost everything.
- Lipase. A baseline matters here specifically because these medications carry a pancreatitis warning, and an isolated elevated lipase six months in is nearly useless without a starting value.
- Iron studies and ferritin, vitamin D, B12. Not glamorous. Frequently the actual answer to "why am I so tired."
I also take a careful history that no lab replaces. Personal or family history of medullary thyroid carcinoma or multiple endocrine neoplasia type 2 is a hard stop for the GLP-1 class, and that comes from asking, not from a tube of blood. The signal behind that warning isn't hypothetical, either. A French nationwide case-control study by Bezin and colleagues, published in Diabetes Care in 2023, found an increased risk of thyroid cancer, including medullary thyroid cancer, in people who'd used GLP-1 receptor agonists for one to three years. The absolute numbers are small and the debate over confounding is genuinely unsettled. But it's exactly why the family history question gets asked out loud, every time, and why routine calcitonin screening is not the answer (it produces more false alarms than it prevents harm).
What We Recheck, and When
The first recheck lands somewhere between eight and twelve weeks, once you've been at a stable dose long enough for the numbers to mean something. After that, we settle into every three to six months depending on what we found and how aggressive your titration has been.
Kidney function is near the top of my recheck list, and for two opposite reasons. Nausea and reduced intake can dehydrate people into a rising creatinine, which is a dosing problem, not a kidney disease. But there's also a real renal upside to this drug class. The FLOW trial, led by Perkovic and published in the New England Journal of Medicine in 2024, found that semaglutide reduced clinically important kidney outcomes and cardiovascular death in patients with type 2 diabetes and chronic kidney disease. So when I watch eGFR, I'm watching for harm and benefit at the same time.
Liver enzymes usually improve. A lot of my patients carry fatty liver they didn't know about, and watching ALT drift down over a year is one of the more satisfying parts of this work. Lipids and A1c get rechecked because they're the point. And I keep asking about right upper quadrant pain, because gallbladder disease is a documented risk. A systematic review and meta-analysis of randomized trials by He and colleagues in JAMA Internal Medicine in 2022 found GLP-1 receptor agonist use was associated with an increased risk of gallbladder and biliary disease, more so at higher doses, longer durations, and when used for weight loss specifically. That last detail matters for exactly the population I see.
One more thing gets tracked that isn't a blood test at all: body composition. Rapid weight loss takes lean mass with it if nobody's paying attention. Scale weight alone will happily tell you a flattering story while you lose muscle you'll want back at seventy.
The Results That Change the Plan
Most panels come back reassuring, and we keep going. But here's what actually moves me off course.
A creatinine that climbs while you're reporting poor intake means we slow the titration and fix hydration before anything else. An asymptomatic lipase bump gets repeated rather than reacted to, because transient elevations happen and one number is not a diagnosis. New right upper quadrant pain with abnormal liver enzymes means imaging, promptly.
An A1c that won't budge despite good adherence tells me we've either got a dosing problem or a diagnosis I've gotten wrong. And a patient whose weight is falling beautifully while their ferritin and B12 crater is a patient who's eating too little of the wrong things, which is a nutrition conversation, not a medication conversation.
Occasionally a result puts us in genuinely gray territory. A mildly reduced eGFR in a sixty-eight year old, say, or liver enzymes sitting stubbornly at twice the upper limit with fatty change on ultrasound. Those aren't automatic disqualifications, and I don't treat them that way. What they are is a reason to start lower, titrate slower, recheck sooner, and put the plan in writing so both of us know what we agreed to. Medicine gets safer when the decision points are settled in advance instead of improvised at month four.
The flip side is worth saying plainly. Sometimes the labs argue for staying the course when the patient wants to quit. Someone six weeks in, frustrated that the scale has stalled, whose A1c dropped a full point and whose triglycerides fell by a third, is succeeding. They just can't see it from the bathroom floor. That's the conversation I most enjoy having, and it's only possible because we have numbers.
It's also worth remembering that the cardiovascular story behind this class came out of exactly this kind of measurement. The SELECT trial, reported by Lincoff and colleagues in the New England Journal of Medicine in 2023, followed more than 17,000 patients with existing cardiovascular disease and overweight or obesity but without diabetes, and found a significant reduction in cardiovascular death, heart attack, and stroke over roughly forty months. Those endpoints matter more than the number on your scale, and they're the reason I keep pulling lipids and A1c long after the weight has plateaued. What Labs Can't Tell You
I want to be careful not to oversell the panel. Labs are a snapshot of chemistry, and you're not chemistry.
No blood test tells me whether you're sleeping. None of them capture whether the nausea is manageable or quietly ruining your week. Nothing on a CMP shows me that you've stopped eating with your family because food has gotten complicated, which is a real and underdiscussed side effect of appetite suppression. Nothing measures whether you're lifting weights, and resistance training is not optional on these medications if you'd like to keep your strength.
Labs also lag behind biology. A1c averages three months, so it can't tell you what happened last week. Ferritin rises with inflammation and can look normal in someone who's genuinely iron deficient. Creatinine depends partly on muscle mass, which is itself changing while you lose weight. Every one of these tests has a personality, and reading them as though they were thermostats is how people end up over-treated.
So the visits matter as much as the draws. In my practice, the labs open the conversation, they don't replace it. I've adjusted plans based on a sentence a patient said on the way out the door far more often than I've adjusted based on a decimal point.
The Short Version
Draw labs before you start. Recheck at eight to twelve weeks, then every three to six months. Know your baseline kidney function, liver enzymes, A1c, lipids, thyroid, and lipase, plus the nutrient panels that explain fatigue. Ask about family history of medullary thyroid cancer before the first dose, not after. And treat body composition and symptoms as data, not as soft extras.

What labs do I need before starting a peptide medication like semaglutide or tirzepatide?
At minimum I want a comprehensive metabolic panel, hemoglobin A1c, a lipid panel, TSH, a complete blood count, and a baseline lipase. I'll usually add ferritin, vitamin D, and B12, because those explain a lot of the fatigue people end up blaming on the medication. Can I start a weight loss injection without any bloodwork?
You can, and plenty of online clinics will happily let you. I just don't think it's good medicine. Without a baseline, there's no way to tell whether an abnormal result six months from now is the medication, your eating, or something that was already there before you started. It also means nobody screened your kidneys, liver, and thyroid first.
Do I need my thyroid checked before starting a GLP-1 medication?
We check TSH, yes, but the more important step isn't a blood test at all. This class carries a warning about medullary thyroid carcinoma and multiple endocrine neoplasia type 2, so I ask about your personal and family history out loud before writing a prescription. Routine calcitonin screening isn't recommended, since it produces more false alarms than real answers.
How often do I need blood work?
Here at Magnolia, we prioritize safety. Typically, we require comprehensive labs every 3 to 4 months during your first year of treatment to dial in your dosage. Once you are stable, we may move to every 6 months depending on your specific health markers.
Are there any side effects or risks with Bloodwork?
The risks of bloodwork are limited to the blood draw itself. Most people get nothing worse than a small bruise or brief soreness at the site, and some feel lightheaded or queasy during the draw, which is why we'll have you sit or lie down if you're prone to that. Rarely, a draw causes a larger bruise, a small clot in the vein, or a temporary nerve twinge in the arm. There's no risk from the testing itself, though results sometimes raise questions that need follow-up testing. Tell us ahead of time if you've had trouble with blood draws before.
