That kind of inflammation is your immune system doing its job, and you'd be in trouble without it. But there's another kind, quieter and far more dangerous, that doesn't announce itself with heat or pain. It smolders in the background for years, and by the time it shows up on your radar it has often already helped write the story of a chronic disease. That slow burn has a name in the research world, and it may be the single most important concept in aging that the average person has never heard of.

And when you trace the big killers of modern life back far enough, you keep bumping into the same suspect. Acute Versus Chronic: Two Very Different Fires

Chronic inflammation is what happens when that response never fully switches off. A landmark 2019 perspective in Nature Medicine by Furman and colleagues laid this out about as clearly as anyone has (Furman et al., 2019). The authors describe how social, environmental, and lifestyle factors can drive what they call systemic chronic inflammation, and how that state feeds directly into the diseases responsible for most death and disability worldwide: cardiovascular disease, cancer, diabetes, chronic kidney disease, fatty liver, and neurodegenerative and autoimmune conditions. One underlying process, a startling number of downstream outcomes. When I first read work like this, it reorganized how I think about prevention.

Inflammaging: Why the Fire Grows With Age

Here's a word worth knowing: inflammaging. It's the observation that as we get older, baseline inflammation tends to creep upward even in people who seem healthy. Researchers coined the term to capture the way chronic, low-grade inflammation and biological aging feed each other.

A thorough 2018 review in Nature Reviews Cardiology by Ferrucci and Fabbri examined this in depth (Ferrucci and Fabbri, 2018). They describe how most older adults develop inflammaging, a condition marked by elevated inflammatory markers that carries a high susceptibility to chronic illness, disability, frailty, and earlier death. The proposed drivers read like a checklist of modern life: central belly fat, increased gut permeability, shifts in the gut microbiome, cellular senescence (those so-called zombie cells that refuse to die and keep spewing inflammatory signals), oxidative stress from tired mitochondria, and lingering low-level infections. What struck me most is their point that the link between inflammaging and cardiovascular disease appears to be causal, not just a bystander association.

That word, causal, is the whole ballgame. If inflammation merely traveled alongside disease, lowering it might do nothing. If it helps cause disease, then lowering it should help prevent it. And that's exactly the question a remarkable clinical trial set out to answer.

The Trial That Made the Case

For a long time, we could only say inflammation was associated with heart disease. Correlation, not proof. Then came a study large enough to test causation head-on. I'm not suggesting that particular medication is the answer for the average person, and it isn't something we use at the clinic. The value of the trial is what it proved about the biology: knock down the inflammation, change the outcome.

The Organs That Pay the Price

One reason chronic inflammation is so insidious is that it doesn't confine itself to a single location. It's systemic, which means it touches nearly every organ, and the damage looks different depending on where it lands.

In the arteries, inflammation helps turn ordinary cholesterol into unstable plaque, which is the mechanism behind most heart attacks and strokes. In the brain, chronic inflammatory signaling is increasingly tied to cognitive decline and the neurodegenerative diseases we dread most in our later years. In the joints, it erodes cartilage and stiffens tissue, feeding the aches that too many people write off as just getting older. In the liver, it drives the fatty, inflamed state that can progress toward scarring. In the pancreas and muscle, it worsens insulin resistance, nudging blood sugar upward and paving the road to type 2 diabetes.

Even mood belongs on this list. When I hear a patient describe a vague, persistent lack of vitality, inflammation is one of the first threads I want to pull on. It rarely acts alone, but it's frequently in the room.

The takeaway isn't to panic every time something aches. It's to recognize that these seemingly unrelated problems, the heart risk and the brain fog and the stiff knees and the stubborn belly fat, may be branches of the same tree. Treat them one at a time and you're forever chasing symptoms. Address the shared root and you get leverage over all of them at once.

So where does this smoldering fire come from in real life? In my practice, the usual contributors are frustratingly ordinary, which is also the good news, because ordinary things can be changed.
