There is a dirty little secret in the world of rapid weight loss that most clinics won't tell you.

You see the headlines. You see the celebrities shrinking before your eyes. You see the patients at Magnolia Functional Wellness dropping 20, 30, or 50 pounds on medications like Semaglutide (Ozempic/Wegovy) and Tirzepatide (Mounjaro/Zepbound).

The results are real. The medication is a miracle for metabolic health.

But there is a catch.If you do this wrong—if you just take the shot and stop eating—you might end up smaller, but you won't end up healthier.

We call it Sarcopenic Obesity. It means you have a "normal" weight on the scale, but your body composition is tragic. You have lost your fat, yes, but you have also stripped away your muscle tissue. You become "skinny fat." Your metabolism crashes. You feel weak. And the moment you stop the medication, the weight comes roaring back because you don't have the muscle engine to burn calories anymore.

As an Internal Medicine Physician certified by the Institute of Functional Medicine, I don't just want you to be smaller. I want you to be metabolically bulletproof. And that means we have to talk about Protein.

The "Ozempic Face" and "Ozempic Butt" Reality

You’ve heard the terms."Ozempic Face" refers to that gaunt, hollowed-out look some people get after rapid weight loss. "Ozempic Butt" is exactly what it sounds like—a flat, sagging rear end.

The media tries to scare you by saying these are side effects of the drug. They are not drug side effects. They are starvation side effects.

When you are in a massive caloric deficit—which GLP-1s make very easy to do—your body needs energy. It will burn fat, yes. But if you aren't giving it enough amino acids (protein), it will also cannibalize your own muscle tissue. It will break down your biceps, your glutes, and yes, the structural muscles in your face, to survive.

If you lose 30 pounds, and 15 of them were muscle, you have aged yourself metabolically by ten years.

Muscle: The Organ of Longevity

In Functional Medicine, we view skeletal muscle as an organ. Actually, it is the Organ of Longevity.

  1. It is your Glucose Sponge: Muscle is the primary place your body stores sugar (glycogen). The more muscle you have, the better your insulin sensitivity. If you lose muscle, you become more insulin resistant, even if you are thinner.
  2. It is your Metabolism: Muscle is expensive tissue. It burns calories just sitting there. Fat is cheap tissue; it burns almost nothing. If you lose muscle, your Resting Metabolic Rate (RMR) plummets. This is why people yo-yo diet.
  3. It is your Armor: As we age, frailty is the enemy. Muscle protects your bones and joints.

So, the goal at Magnolia Functional Wellness is simple: Burn the Fat, Keep the Muscle.

The Magnolia Protein Protocol

How do we do this? We have to treat your nutrition with the same precision as your medication.

When you are on a GLP-1, your appetite is suppressed. You might only be hungry for 1,200 calories a day. But if those 1,200 calories are crackers, salad, and wine, you are in trouble.

You have to prioritize protein. Here is the math we use at the clinic:

The Golden Rule: Aim for 0.8 to 1 gram of protein per pound of your Ideal Body Weight.

If your goal weight is 150 lbs, you need to eat ~150 grams of protein a day.That sounds like a lot. Especially when you aren't hungry. But it is non-negotiable.

How to Execute This:

  1. Protein First: When you sit down to eat, eat the chicken, the steak, the eggs, or the fish first. If you get full and leave the potatoes on the plate, fine. But don't fill up on bread and leave the protein.
  2. Supplements are Okay: If you physically can't chew that much chicken, use a high-quality whey or plant protein shake. It’s an easy 30 grams.
  3. Timing Matters: Try to get 30 grams of protein within an hour of waking up. This sets your metabolic tone for the day and stops muscle breakdown after the night's fast.

Resistance Training: The Signal

Eating protein gives you the bricks to build the house. But Resistance Training gives the order to build it.

You cannot just do cardio.If you are on Semaglutide and you only do Peloton or walking, you will lose muscle.

You have to lift heavy things. You have to send a signal to your body that says: "Hey, we need these muscles! Do not burn them for fuel!"

I tell my patients in Southlake: I don't care if it’s Pilates, CrossFit, or old-school dumbbells in your garage. You need to create tension in the muscle at least 2–3 times a week.

What About Creatine?

This is a functional medicine tip I give almost everyone. Creatine Monohydrate (5 grams a day) is not just for bodybuilders. It is one of the most researched supplements for preserving lean muscle mass and improving cognitive function. It helps hydrate the muscle cell.

It is cheap, safe, and tasteless. Put it in your morning water. It’s an insurance policy for your muscles.
